Dr. Drew Marano, MD is a fellowship-trained plastic and reconstructive surgeon whose practice focuses on Phalloplasty and other gender-affirming surgeries. Based in New York City and Livingston, New Jersey, Dr. Marano provides advanced surgical care grounded in technical excellence and genuine compassion. After completing specialized fellowship training in gender-affirming surgery, he joined Dr. Jonathan Keith at East Coast Advanced Plastic Surgery. In 2025, Dr. Marano and Dr. Keith co-founded GAMMA (Gender Affirming Medical and Microsurgical Associates), a pioneering practice dedicated to advancing the art and science of gender-affirming surgery. Renowned for his meticulous surgical technique and collaborative approach, Dr. Marano provides an affirming environment for patients pursuing Phalloplasty and other gender-affirming surgeries throughout the New York metro area and northern New Jersey.

Dr. Marano is a classically-trained pianist who earned his bachelor’s degree in Music from Brown University. While it might surprise you, musical training as a precursor to medical school is actually pretty common. In fact, medical schools actively recruit high-end musicians. Surely, the hand and finger dexterity of a pianist would be an advantage in the operating room but it goes deeper than this. Musicians know how to learn skills and maintain them. Dedication to practice and constantly striving for excellence are absolutely traits that you want in a surgeon, especially one who is performing Phalloplasty, one of the most complex and demanding surgeries in existence. Musicians are also excellent listeners, another quality of a great surgeon.

After medical school and a six-year intensive plastic and reconstructive surgery residency, Dr. Marano sought out focused training in gender surgery at Harvard University. He became the first-ever fellow of the Harvard University Complex Gender and Microsurgery fellowship program directed by Dr. Devin O'Brien Coon.

Phalloplasty Techniques

Dr. Marano is trained to perform all of the popular methods of Phalloplasty, such as RFF and ALT as well as MLD, Abdominal (with or without urethral lengthening) and groin flap Phalloplasty. If you're unsure about which method to pursue, Dr. Marano will clarify the pros and cons of each and how they relate to your goals during your consultation.

To some degree, Dr. Marano is able to customize the approach to your surgery. For example, if you're interested in non-binary bottom surgery, he can perform Phalloplasty without Vaginectomy. However, if urethral lengthening is also desired, he requires a Vaginectomy to reduce the risk of urethral complications.

To create the scrotum, Dr. Marano uses rotated labial flaps ("V-Y Scrotoplasty"). Patients can choose between saline- or silicone-filled testicular implants. For erectile implants, Dr. Marano uses semi-rigid/malleable, 2-piece and 3-piece inflatable devices.

Dr. Marano’s team includes Reconstructive Urologist Dr. Karl Coutinho, who assists with urethral lengthening and implantation of testicular and penile implants. Dr. Coutinho also offers repair surgeries for urological complications following Phalloplasty.

Dr. Marano operates in both in New York City and the greater Newark area, at Lenox Hill Plastic Surgery Center in NYC, and Short Hills Surgery Center and North Fullerton Surgery Center, both in New Jersey.

Surgery Requirements
Dr. Marano is a member of the World Professional Association for Transgender Health (WPATH) and follows WPATH guidelines for surgery:

  • You will need to provide two referrals from qualified mental health professionals.
  • Age 18+

Insurance Accepted
Dr. Marano is in-network with Horizon Blue Cross Blue Shield of New Jersey, and can also accept out-of-area BlueCard members, and some New Jersey Medicaid plans. (Credit and debit cards are also accepted.) New York Medicaid and Medicare are not accepted.
