Dr. Toby Meltzer, MD is a plastic and reconstructive surgeon who is globally recognized as one of the leading surgeons in the field of Gender Affirming Surgery. For many years, Dr. Meltzer was one of very few surgeons in the U.S. offering Phalloplasty. He became the first surgeon in Arizona to specialize in Gender Surgery when he moved from Portland, Oregon to Scottsdale to expand his practice 2003.
Dr. Meltzer offers three types of Phalloplasty:
- Radial Forearm Flap (RFF) Phalloplasty: The creation of a phallus by using skin, nerves and vessels from the forearm. This is a microsurgical procedure that creates a sensate penis of adequate size for intercourse.
- Pedicled Groin Flap Phalloplasty: The creation of a phallus by using skin harvested from the groin and forming a tube that is slowly released from the patient's body. This is a staged procedure that creates a penis of adequate size but without erogenous sensation.
- Groin/RFF Composite Phalloplasty: Combines groin flaps for creation of the phallus and a forearm flap to create the urethra.

Surgery Requirements
Dr. Meltzer follows WPATH guidelines for surgery. You will need to provide two referrals from qualified mental health professionals for Phalloplasty. plus a letter from your hormone specialist stating you have been on 12 months of continuous testosterone replacement therapy, unless clinically contraindicated.
Financing & Insurance
Dr. Meltzer accepts cash, credit cards and medical financing. He contracts with Kaiser, is in-network with BCBS AZ, and has successfully billed UHC, Aetna, Cigna and Harvard Pilgrim insurance. Medicaid, Medicare and Tricare are not accepted.

NOTE: Dr. Meltzer is a regular presenter at transgender conferences, including First Event, Keystone and Southern Comfort. Dr. Meltzer often does consultations at these conferences, offering a convenient way for patients from around the country to meet with him without having to travel to Arizona.
Current wait list for a surgery date: 12 months for Groin Phalloplasty; 24 months for microsurgical Phalloplasty.
